- I submitted 3 comprehensive H&Ps and presented 2 of them in my site evaluation for LTC
- I also presented 10 pharm cards, discussing the most relevant information of each drug
- I presented 1 Journal Article relevant to one of my H&P
- Site evaluator commented that my second H&P presentation was better that the first one, it was more organized and fluent, but I still need to work on many points.
- I plan to work on writing a better and thorough discharge plans that should include all the disciplines pertinent to the patient
- I should improve on my differential diagnosis list and able to question why the clinicians who assess the patient had that particular diagnosis
- I plan to work on writing better comprehensive H&Ps and when it comes to presenting them, I should present only the relevant information
- I plan to concentrate more on writing better HPI, in the LTC it should tell the “whole story from home to hospital to the moment I see the patient”
- I plan improve and express clearly what I am documenting, it has to be consistent and pertinent to patient’s physical exam
- “When I write up a patient, I need think about what are the most important questions/issues and make sure that they have been addressed”
